MDM Based

A 59-year-old patient’s diabetes and hypertension are stable.  Physician ordered EKG and angiogram.  Physician considered minimal risk of morbidity in this patient.  Identify the level of MDM for this established patient during the office visit

 An office consultation is requested by PCP with a referral to a cardiologist for a 48-year-old patient with his chronic illnesses namely diabetes and hypertension are stable.  Physician ordered EKG, Chest X-ray and angiogram.  Physician considered high risk of morbidity in this patient.  Identify the level of MDM for this patient

Inpatient or observation consultation Established patient home or residence visit with moderate level MDM

On day 3, attending physician provided moderate level of MDM in the observation care unit

On day 2, cardiologist provided moderate level of MDM in the observation care unit

A physician provided E/M service to a new patient in campground with moderate level of MDM

Established patient home or residence visit with moderate level MDM
